Med Pass Reminders
Heart Harmony Healthcare LLC provides medication reminder assistance as a non-medical support service. Our caregivers may remind clients when it is time to take medications, assist with reading medication labels, and document that reminders were provided. We do not prescribe, administer, dispense, inject, alter dosages, or make medical decisions regarding medications. Clients remain responsible for taking their own medications unless otherwise authorized under applicable law or through a licensed healthcare professional. Any concerns or observed changes are promptly reported to the client, authorized representative, or appropriate healthcare provider.

Medication Reminder Assistance Includes:
- Timely reminders according to the client’s prescribed medication schedule.
- Verbal reminders when medications are due.
- Assistance locating properly stored medications.
- Reading medication labels upon client request.
- Opening medication containers when physically appropriate.
- Providing water or beverages with medications, when requested.
- Reminding clients of dosage times only—not dosage instructions.
- Documenting medication reminder completion in the care record.
- Encouraging medication compliance without coercion.
- Observing for missed or forgotten medication reminders.
- Reporting missed doses to authorized family members, when approved.
- Reporting medication concerns to supervisors immediately.
- Maintaining confidentiality of all medication information.
- Following the client’s individualized care plan.
- Encouraging clients to ask healthcare providers medication-related questions.
- Assisting with medication schedule organization, when appropriate.
- Never changing medication dosages or schedules.
- Never administering injections or prescription treatments.
- Never providing medical advice regarding medications.
- Always operating within Texas law, company policy, and non-medical caregiver scope of practice.
